YOUTH REGISTRATION BEGINS HERE.  Follow Steps 1, 2, 3 and 4.
ALL J.O. PLAYERS (Youth) must be individually registered with ASA to participate in Championship Tournament Play. 
Download the "J.O. Individual Registration Form" below, fill it out completely, attach a check or money order for all players and all coaches X $8.00 each.  Make the check or money order payable to Mississippi ASA.  This should be mailed to your local District Commissioner- if you do not know who your commissioner is, please check under the District Commissioners tab or mail to the Mississippi ASA office.
 INDIVIDUAL REGISTRATION DOES NOT APPLY TO ADULT PLAYERS.  

ADULT REGISTRATION BEGINS HERE. Follow Steps 2, 3 and 4.
Download the "Championship Tournament Entry Form" below in order to enter a team into Championship Tournament Play.  All J.O. Teams (Youth) and Adult teams must complete this form to play.  Fill the form out completely and attach a check or money order for the correct Tournament Entry Fees due for this tournament.  Make the check payable to Mississippi ASA and mail to your local District Commissioner or to the Mississippi ASA office.
OFFICIAL TEAM ROSTER:   This roster is not needed until your team participates in their first Championship Tournament game.  All J.O. Teams (Youth) and Adult teams must complete this form to play.  Download a copy of the "Official Team Roster" below, have it filled out properly and signed by the team manager or coach.  You should turn this roster in to the Tournament Director when you take your first game's line-up card to the pressbox of the tournament site.  
PLAYER IDENTIFICATION INFORMATION:    The following pertains to player identification that must be presented before playing your first Championship Tournament Game.  Each player must provide proper I.D. (which shall include a current photograph and the player's signature) or the player shall be declared ineligible and the team subject to suspension.  For this purpose, driver's licenses or comparable identification is acceptable.  For each member of a J.O. team, one of the following forms of birth verification or proof of age must be available to the Tournament Director: Birth certificate, hospital certificate, baptismal certificate, driver's license, a photo I.D. issued by the local ASA Commissioner and/or their designated deputy or a valid passport.  Photocopies of any of the above are acceptable for J.O. identification.
